CGHS Demand Survey

Dear Comrades,

I am drawing all of yours attention towards the point regarding the medical fecility to all the Central Govt. Employees. In this connection here I am enclosing the Demand survey issued by the Ministry of Health & Family Survey and request all of you to please fill up the format so as to enable us for demand of CGHS in Surat City also. The format is as under:





So all of you are requested to send the form before 15.02.2010 and make this move to a success one.
